Reminder re Allergies and Medical Information

A reminder to all of our families that if your child has an allergy, a medical condition, and/or requires an Epi-Pen or medication to be administered in school, that you must provide us with the appropriate updated medical forms each school year. It is also important to let us know of any changes throughout the school year.

You can do this by informing your child’s Homeroom teacher or by calling the school. Please see this link for a complete list of all of the relevant medical and allergy forms you may need. https://mdjh.info/medical-forms/
